Origin: medial margin of the olecranon and upper two-thirds of the dorsal border of the ulna by an aponeurosis.

Insertion: pisiform bone, hamatum bone, pisihamate ligament, pisometacarpal ligament, pisometacarpal ligament.

Between the two heads passes the ulnar nerve and ulnar artery.
